Role of Intrapartum Ultrasound in Modern Obstetrics

چکیده

Intrapartum ultrasound has become more popular over the last decade. It provides a more objective assessment of fetal head position and station than traditional digital vaginal examination, which is now known to be inaccurate. Correct identification of fetal head position is particularly useful before instrumental delivery, although it should not be used in the first stage of labour to predict successful vaginal delivery. Fetal head station can be assessed by determining the relationship between maternal and fetal structures transperineally, such as head-perineum distance and angle of progression. Intrapartum ultrasound has potential use before and during labour. Current evidence, however, shows it is most useful during the second stage of labour, especially when choice of optimal mode of delivery is not apparent. We believe that intrapartum ultrasound is a promising diagnostic tool that will ultimately improve the wellbeing of mothers and babies. Hong Kong J Gynaecol Obstet Midwifery 2017; 17(2):134-40

Intrapartum sonography to determine fetal occipital position: interobserver agreement.

OBJECTIVE To investigate the interobserver agreement on the intrapartum ultrasonographic definition of the fetal occipital position. METHODS In 60 singleton pregnancies in labor at term the fetal occipital position was determined by transabdominal ultrasound by two appropriately trained sonographers who were not aware of each other's findings. The Bland-Altman plot was performed and the 95% l...

Intrapartum sonographic assessment of labor.

Fetal head progression during labor is difficult to assess. Digital examination has been shown to be an inaccurate method. Utilizing the ultrasound technology in the delivery room can standardize the way we assess head progression. Intrapartum ultrasound is applicable, for the assessment of the progression of labor and assists the obstetrician, in decision making regarding the need for an assis...

Intrapartum ultrasound: A useful method for evaluating labor progress and predicting operative vaginal delivery

The last step of a successful pregnancy is the safe delivery of the fetus. An important question is if the delivery should vaginal or operative. In addition to the use of conventional antenatal ultrasound, the use of intrapartum ultrasound to evaluate fetal head station, position, cervical ripening, and placental separation is promising. This review evaluates and summarizes the usefulness of in...

Comparison of transvaginal digital examination with intrapartum sonography to determine fetal head position before instrumental delivery.

OBJECTIVE To investigate the accuracy of intrapartum transvaginal digital examination in defining the position of the fetal head before instrumental delivery. PATIENTS AND METHODS In 64 singleton pregnancies undergoing instrumental delivery the fetal head position was determined by transvaginal digital examination by the attending obstetrician. Immediately after or before the clinical examina...
